Program Description

The Arizona Medicaid Program provides medical benefits to low income people who have no medical insurance or have inadequate medical insurance. The Federal government establishes general guidelines for the administration of Medicaid benefits. The Arizona Medical Assistance Program provides a full range of medical services to an individual or family who meet certain eligibility requirements. The Arizona Medicaid medical benefits are provided by the Arizona Health Care Cost Containment System (AHCCCS).

General Medicaid Program Requirements

In order to qualify for this benefit program, you must be a resident of the state of Arizona, a U.S. national, citizen or permanent resident in need of health care/insurance assistance, whose financial situation would be characterized as low income or very low income.

To download and print an "Application for Assistance", visit https://egov.azdes.gov/cmsinternet/appforms.aspx. Under "Which Category", select "Medical Assistance" from the drop-down menu in Option 1. Of the available options, click the one that meets your needs: English-Large Print, Spanish or Spanish-Large Print.

Veterans Affairs Aid & Attendance and Housebound

The Department of Veteran Affairs offers a special benefit to war era veterans and their surviving spouses called Aid and Attendance. This is a tax-free benefit designed to provide financial assistance to help cover the cost of long term care in the home, in an assisted living facility or in a nursing home. This Benefit is for those veterans and surviving spouses who require the regular attendance of another person or caregiver in at least two of the daily activities of living such as transportation, dressing, undressing, bathing, cooking, eating, grooming, personal hygiene needs, homemaking and maintenance.

 
If you qualify:

Single veteran maximum benefit is 1,094 per month
Married Veteran maximum benefit is 1,703 per month
Veteran and Veteran Spouse maximum benefit is 2,019 per month
Surviving Spouse (not a veteran) maximum benefit changes from year to year.
 
Because these are VA benefits the income received is not included as income for any other government program. Income and Net worth requirements must be proven to the VA.  
·         A veteran alone must have “countable income” LESS than $14,457 a year*
·         A veteran with a spouse must have “countable income” LESS than: $18,120 a year*
 
*Countable Income: The non-reimbursed cost of Medical Care and Medicaid Assisted Living Care could offset income and asset requirements. Therefore, someone who exceeds the annual income listed or net worth limits listed above but has high non-reimbursed medical and/or assisted living costs should still apply and may still qualify.

There are several organizations that will help complete the forms by law no one can charge a fee to prepare these forms, therefore, many organizations perform the services either without compensation or they provide an optional service that earns them a fee to offset their costs.
